Subject: Physicals for Camp and Philmont
Physicals will be performed at Blue Springs Family Care 104 N 7 highway on Thursday April 19th from 6:00-8:00 PM.
Cost: $10 cash checks to Dr. Mackey
What to bring: Immunization/Vaccination Record,
Insurance card (for copying only),
Emergency contact information
Who: All scouts and Adults going to philmont must be there that night or make arrangements otherwise with Dr. Mackey. Even if you have had a physical recently with your physicain, It will never hurt anyone to have another physical. Only this form can turned in, and it needs to be completed, signed by both the parents and the scout and the copy of the insurance card all needs to be completed that night so these can be mailed back to Philmont as soon as possible.
All other scouts (The only scouts that do not need physicals are 2nd or 3rd year scouts. All 4th year and older will need a new physical this year) and Adults that are going to camp are also welcome that night. There will be 3 doctors doing physicals. Dr. peterson, Dr. Mackey, and Dr. Gunn
Tetanus: Holly Duboc from the Health Department will also be there and she will be administering (free of charge) a tetanus vaccination booster (Tdap) to all scouts and Adults who need one. Please make sure to bring your Shot record with you.
